The Retired Membership is for nurses who have retired from the specialty of public health nursing as described in the Regular Membership but no longer derives income from public health nursing employment.
Payment Options
- Process your application entirely online, paying securely via Paypal (optionally by Credit Card). You can pay for your membership and submit for reimbursement if it is allowed by your community/township/organization.
- Process your application entirely online and select an invoice as your payment option. You will receive an invoice via email with payment pending (to the email address you provided on your application) for submission to your community/township/organization which can be mailed in with your check to MAPHN PO Box 537 Milton, MA 02186. Your community may require a W-9 from MAPHN.
ALL MEMBERSHIP APPLICATIONS ARE DONE ON LINE or THROUGH INVOICING. There are no paper form registrations. Membership is not transferable. Some exceptions may apply but requires an approval from the Executive Committee.
